Responsibilities

• Work collaboratively with other healthcare providers to deliver comprehensive care.

• Assist with physical therapy exercises as instructed.
• Provide respite for family caregivers by taking over care duties.
• Monitor changes in clients' conditions and report to the Care Manager.
• Maintain confidentiality and respect client privacy.
• Engage clients in meaningful conversations to enhance their emotional well-being.
• Create a safe and supportive environment for clients.

Requirements

Education

• High school diploma or equivalent
• Certification in Health and Social Care

Experience

• At least 1 year of experience in a similar role preferred
