Job Description
We are seeking a highly skilled Pharmacist to join our dedicated team in Manchester. The ideal candidate will play a vital role in ensuring safe and effective medication use, providing consultations to patients, and managing pharmaceutical services.
Responsibilities
Review patient records and assess prescription needs.
Advise patients on medication, including side effects and interactions.
Engage with patients to answer questions and provide education on health topics.
Work with healthcare teams to develop and implement pharmacy protocols.
Document patient consultations and maintain accurate medical records.
Conduct medication therapy evaluations and adjust therapies as appropriate.
Manage pharmacy operations in absence of the Pharmacy Manager.
Assist in developing and implementing quality improvement initiatives.
Supervise pharmacy technicians and assist with their training.
Facilitate collaboration with community resources to support patient care.
Advantageous
Experience with clinical pharmacy practice: Practice in directing clinical activities and enhancing patient care.
Familiarity with electronic health records (EHR): Proficient in using EHR systems for managing patient information.
