Job Description

As an Independent Living Specialist, you will provide crucial support to individuals who need assistance in maintaining their independence. Your role will involve assessing needs, developing tailored care plans, and collaborating with other healthcare professionals.

Responsibilities

  • Conduct needs assessments for individuals in various settings.
  • Create and update care plans based on individual assessments and feedback.
  • Work alongside families to support client goals and strategies.
  • Advocate for clients' needs and rights within the healthcare system.
  • Provide training on assistive technologies to individuals and caregivers.
  • Keep meticulous records of interactions and outcomes.
  • Participate in team meetings to share insights and improve practices.
  • Engage in ongoing professional development and training.
  • Uphold confidentiality and professionalism in all client dealings.
  • Assist in product evaluations and feedback collection for assistive technologies.

Requirements

Education
  • Bachelor's degree in a relevant field (e.g. Occupational Therapy, Psychology)
  • Registration with NMC or SWE is preferred
Experience
  • 2+ years of experience in community support or healthcare roles
