Responsibilities

• Conduct medication reviews for patients to ensure optimal therapy.
• Advise patients on lifestyle changes to improve health.
• Implement pharmacy policies and procedures.
• Assist with the management of chronic diseases through medication therapy management.
• Participate in community health initiatives.
• Provide immunizations and other health services as needed.
• Collaborate with multidisciplinary teams for comprehensive patient care.
• Document all patient consultations accurately.

Requirements

Education

• Doctor of Pharmacy (PharmD) preferred

Experience

• 3+ years in a pharmacy or healthcare setting

• Clinical Pharmacy Skills

Certifications

• Clinical Pharmacy Certification

Languages

• English: Fluent
• Additional language is a plus: Conversational

• Specialisation in a therapeutic area: Specialisation in areas such as oncology, cardiology, or pediatrics is an advantage.
