Job Description

As a Home Visitor, you will play a vital role in providing home-based support to clients. Your responsibilities will include assessing needs, creating care plans, and ensuring clients receive the guidance necessary for their health and well-being.

Responsibilities

• Perform assessments during home visits to identify the needs of clients.
• Develop individualized care plans that address the specific needs of each client.
• Deliver high-quality in-home care services that promote health and well-being.
• Communicate regularly with clients and their families to keep them informed and engaged in the care process.
• Coordinate with in-home care specialists to enhance service delivery.
• Participate in team meetings to share insights and best practices.
• Manage any necessary paperwork and ensure compliance with organisational policies.

Requirements

Education

• High school diploma or equivalent
• Diploma in a health-related field is preferred

Experience

• 1-2 years of experience in social services, healthcare, or related field

Technical Skills

• Microsoft Office Suite
• Electronic Health Record (EHR) software

Soft Skills

• Excellent communication

Certifications

• DBS check

Languages

• English: Fluent
