Job Description
As a Home Visitor, you will play a vital role in providing home-based support to clients. Your responsibilities will include assessing needs, creating care plans, and ensuring clients receive the guidance necessary for their health and well-being.
Responsibilities
Perform assessments during home visits to identify the needs of clients.
Develop individualized care plans that address the specific needs of each client.
Deliver high-quality in-home care services that promote health and well-being.
Communicate regularly with clients and their families to keep them informed and engaged in the care process.
Coordinate with in-home care specialists to enhance service delivery.
Participate in team meetings to share insights and best practices.
Manage any necessary paperwork and ensure compliance with organisational policies.
Requirements
Education
High school diploma or equivalent
Diploma in a health-related field is preferred
Experience
1-2 years of experience in social services, healthcare, or related field
