Job Description

As a Healthcare Social Worker, you will play a crucial role in delivering mental health services to individuals and families in need. You will conduct assessments, provide therapy and work collaboratively with a team of healthcare professionals to ensure comprehensive care.

Responsibilities

  • Perform intake assessments and evaluations for new clients seeking mental health support.
  • Design and update treatment plans based on client progress and feedback.
  • Facilitate support groups to foster community and shared experiences among clients.
  • Ensure all client interactions are documented accurately in electronic health records.
  • Advise clients on coping strategies and resources to aid their mental health journey.
  • Participate in peer reviews and case consultations to enhance service quality.
  • Develop partnerships with local organisations to improve community health outcomes.

Requirements

Education
  • Master's degree in Mental Health or a related field
Experience
  • Experience in both in-person and telehealth services
