Job Description

We are seeking a skilled and compassionate Home Health Physical Therapist (Home Health PT) to join our team. As a Home Health PT, you will provide physical therapy services to patients in their homes, ensuring their optimal physical functionality and well-being.

Responsibilities

  • Conduct thorough patient assessments to determine therapy needs.
  • Develop and implement effective treatment strategies for patients in their homes.
  • Monitor progress and adapt treatment plans based on patient feedback.
  • Provide guidance to families about rehabilitation exercises and techniques.
  • Work in tandem with interdisciplinary teams to deliver holistic care.
  • Document treatments and patient interactions in EMR systems.
  • Participate in care planning with nursing and physician teams.
  • Ensure compliance with health and safety standards in all practices.
  • Engage in continuous learning and adapt practices based on new evidence.
  • Foster supportive relationships with patients to encourage rehabilitation.

Requirements

Education
  • Bachelor's degree in Physical Therapy or related field
  • Master's degree preferred
Experience
  • A minimum of 2 years' experience in home health or community-based physical therapy.
