We are seeking a skilled Primary Care Nurse Practitioner (PCNP) to join our healthcare team. As a PCNP, you will provide quality primary care services to patients, working collaboratively with physicians and other healthcare professionals. Job Responsibilities
  • Perform comprehensive assessments to determine patients' healthcare needs.
  • Manage health maintenance and disease prevention strategies.
  • Collaborate with multi-disciplinary teams to ensure optimal patient care.
  • Document patient interactions in the electronic health record (EHR).
  • Participate in developing and implementing health education initiatives.
  • Assist in care coordination and follow-up for patients with chronic conditions.
  • Conduct minor procedures as required.
  • Engage in continuous professional development.
  • Provide advice and support for patients and families.
  • Evaluate patient progress and modify treatment plans as necessary.
Job Requirements
  • Valid nursing license in the UK
  • Completion of an accredited Nurse Practitioner program
  • Excellent clinical assessment skills Expert in clinical assessments and diagnostics.
  • Proficiency in EHR systems Familiar with various Electronic Health Record systems.
  • Knowledge of chronic disease management Strong background in managing chronic conditions.
  • Strong communication skills Ability to communicate effectively with patients and colleagues.
  • Empathy and compassion Demonstrated empathy in patient interactions.
  • Basic Life Support (BLS) certification
  • Advanced Life Support (ALS) certification
  • English (Fluent)
