Join our healthcare team as a Primary Care Nurse Practitioner (PCNP) where you will be responsible for delivering exceptional primary care services. You will collaborate closely with physicians to implement comprehensive patient care plans while ensuring high standards of care are maintained.

Responsibilities

• Perform comprehensive assessments to determine patients' healthcare needs.
• Manage health maintenance and disease prevention strategies.
• Collaborate with multi-disciplinary teams to ensure optimal patient care.
• Document patient interactions in the electronic health record (EHR).
• Participate in developing and implementing health education initiatives.
• Assist in care coordination and follow-up for patients with chronic conditions.
• Conduct minor procedures as required.
• Engage in continuous professional development.
• Provide advice and support for patients and families.
• Evaluate patient progress and modify treatment plans as necessary.

Requirements

Education

• Registered Nursing qualification from an accredited institution
• Specialization in Primary Care or relevant field

Experience

• Experience in a primary care setting preferred.

Certifications
