Job Description

We are looking for a dedicated Nurse Practitioner to join our team in Bristol. The ideal candidate will have extensive experience in clinical settings and a passion for delivering high-quality patient care. In this role, you will assess and manage patients, provide education, and collaborate with multidisciplinary teams.

Responsibilities

• Manage patient care in accordance with established protocols and guidelines.
• Facilitate referrals to specialists when necessary.
• Conduct follow-up visits to monitor patient progress.
• Provide telehealth consultations as needed.
• Advocate for patient needs and coordinate care across services.

Requirements

Education

• Master's degree in Nursing or related field
• Nurse Practitioner diploma or equivalent

Experience

• 3-5 years of relevant nursing experience

Certifications

• Nurse Practitioner certification

Languages

Advantageous

• Experience with electronic health records (EHR): Familiarity with EHR systems for patient documentation.
