Job Description

We are seeking a highly skilled and compassionate Registered Nurse (RN) Case Manager to provide efficient and effective medical care coordination for patients. The successful candidate will be responsible for ensuring high-quality healthcare services, effective communication, and smooth patient transitions throughout the treatment process.

Responsibilities

  • Conduct comprehensive assessments of patient needs and develop suitable care plans.
  • Facilitate effective communication among healthcare team members and ensure continuity of care.
  • Monitor patient progress and adjust care plans as necessary based on patient feedback and clinical outcomes.
  • Guide patients in understanding their health conditions and managing their care.
  • Oversee the management of patient records in compliance with relevant regulations and best practices.
  • Engage in community outreach and education to enhance patient awareness and involvement in their care.

Requirements

Education
  • Bachelor's degree in Nursing (BSN)
Experience
  • Experience in managing complex medical cases or chronic diseases.
Technical Skills
  • Detail-oriented and able to maintain accurate records
  • Flexibility to adapt to changing patient needs and work demands
Soft Skills
  • Ability to work effectively in a team and independently
Languages
  • English: Fluent

Advantageous

  • Fluency in additional languages: Ability to communicate effectively with diverse patient populations.
  • Experience in palliative care or end-of-life support: Knowledge and experience in providing sympathetic care for terminally ill patients.
