Responsibilities
Deliver pharmaceutical care by assessing patient needs.
Communicate effectively with patients regarding medication usage.
Implement medication therapy management and follow-up.
Maintain drug formulary and stay updated on new medications.
Attend pharmacy meetings and contribute to team goals.
Monitor patient progress and ensure medication effectiveness.
Support a culture of compliance and ethical practice in the pharmacy.
Encourage patient education and health promotion activities.
Evaluate pharmacotherapy regimens for improvement opportunities.
Requirements
Education
Master’s in Pharmacy or related field is preferred
Experience
Minimum 3 years in a community or hospital pharmacy setting
