Job Description

The Palliative Care Nurse Practitioner plays a vital role in providing holistic care to patients with life-limiting illnesses, ensuring a dignified and supportive end-of-life experience. You will be instrumental in symptom management and care coordination, working closely with multidisciplinary teams.

Responsibilities

  • Asses patients to ensure that all their care needs are met comprehensively.
  • Implement effective symptom management strategies and provide interventions as necessary.
  • Work alongside a team of healthcare professionals to ensure quality care delivery.
  • Support families in understanding the palliative care process and addressing their concerns.
  • Engage in continuous educational opportunities to enhance palliative care knowledge.
  • Document care interventions in a timely and accurate manner to uphold best practices.
  • Provide training and support to emerging healthcare professionals in palliative care.
  • Maintain connections with community resources for additional patient and family support.
  • Participate in case conferences and contribute positively to multidisciplinary discussions.

Requirements

Education
  • Registered Nurse (RN) qualification with valid NMC registration
  • Master’s degree in Nursing focusing on Palliative Care preferred
Experience
  • Experience in independent practice as a Nurse Practitioner in Palliative Care
Technical Skills
  • Proficiency in electronic health records (EHR)
  • Knowledge of telehealth platforms
Soft Skills

Certifications
  • Certification in Palliative Care
Languages
  • English: Fluent
