Job Description

We are seeking a dedicated pharmacist to join our team in Swansea. You will play a vital role in providing excellent pharmaceutical care, ensuring patients receive the correct medications, and supporting health initiatives. Your expertise will help enhance the well-being of our community.

Responsibilities

  • Assess patient medications for efficacy and safety.
  • Create and implement patient medication management plans.
  • Provide recommendations on over-the-counter medications.
  • Participate in quality improvement projects within the pharmacy.
  • Utilize technology to manage pharmacy operations effectively.
  • Lead medication therapy management sessions for patients.
  • Collaborate with local healthcare providers for integrated care.

Requirements

Education
  • PharmD or equivalent doctoral degree
  • Postgraduate qualifications in clinical pharmacy are advantageous
Experience
  • 3+ years of experience in a pharmacy leadership role
