Job Description

We are seeking a passionate and qualified Diabetes Educator to join our team. The Diabetes Educator will be responsible for providing education and support to individuals diagnosed with diabetes, helping them effectively manage their condition and improve their overall quality of life.

Responsibilities

  • Assess individual patient needs and tailor educational interventions accordingly.
  • Create and implement workshops that promote effective diabetes management.
  • Support patients in setting and achieving health-related goals.
  • Review and explain diabetes care technologies and tools to patients.
  • Use data from diabetes management software to track patient progress.
  • Advocate for patients' needs within the healthcare team.
  • Monitor patients' responses to educational interventions and follow up as needed.
  • Develop strategies to enhance patient motivation and compliance.
  • Stay abreast of NHS guidelines and compliance requirements.
  • Engage in continuous professional development to refine teaching methods.

Requirements

Education
  • Bachelor's degree in Nutrition, Public Health or a related field
Experience
  • At least 2 years experience working with diabetes patients
