Healthcare Social Worker (Discharge Planner)

Job Description

As a Healthcare Social Worker, specifically a Discharge Planner, you will be responsible for coordinating and facilitating safe and timely discharges of patients. This role is crucial in ensuring continuity of care and enhancing patient satisfaction post-discharge.

Responsibilities

• Coordinate the discharge processes to ensure a smooth transition from hospital.
• Evaluate patient's progress and update discharge plans as necessary.
• Liaise with other healthcare providers to facilitate a continuum of care.
• Train and support junior staff and interns in discharge planning and patient care.
• Monitor and evaluate the effectiveness of discharge plans through follow-up.

Requirements

Education

• Master's degree in Nursing or Healthcare Administration
• Additional training in social work is advantageous

Experience

• 3-5 years of experience in healthcare or case management
