Job Description

We are currently seeking a dedicated Registered Nurse in the role of Discharge Planner. The successful candidate will be responsible for ensuring safe and efficient transitions of care for patients being discharged from hospital. This role requires a compassionate approach coupled with strong organisational skills to coordinate discharge plans effectively.

Responsibilities

  • Ensure that appropriate community resources and services are available for patients after discharge.
  • Participate in quality improvement initiatives to enhance discharge processes.
  • Liaise with community organisations and service providers to improve patient support.
  • Monitor patients' progress post-discharge and adjust care plans as necessary.
  • Adhere to relevant regulations and guidelines in discharge planning.

Requirements

Education
  • Registered Nurse with current UK Nursing and Midwifery Council (NMC) registration
  • Diploma or degree in Nursing
Experience
  • At least 2 years of experience in a healthcare setting, preferably in discharge planning.
