Job Description

We are seeking a dedicated Social Worker to join our Liverpool team, where you will play a crucial role in supporting vulnerable individuals and families. In this position, you will conduct assessments, develop care plans, and work collaboratively with other professionals to ensure individuals receive the necessary support.

Responsibilities

• Evaluate clients' situations through assessments and interviews.
• Establish care plans that address clients' unique needs and goals.
• Facilitate access to community resources and services.
• Work closely with other professionals to coordinate care.
• Provide regular reports on client progress and outcomes.
• Ensure compliance with relevant policies and regulations.
• Foster relationships with clients and their families to support progress.
• Lead workshops or sessions on life skills and self-advocacy.
• Promote public awareness of social issues and community resources.

Requirements

Education

• Bachelor's degree in Social Work
• Master's degree in Social Work (preferred)

Experience

• 3+ years of experience in a social work setting
