Job Description

We are looking for a compassionate and dedicated Social Worker to join our team in Middlesbrough. This role involves supporting individuals and families through various challenges, including mental health issues, substance abuse, and family dynamics. The successful candidate will develop care plans, coordinate with healthcare providers, and advocate for client needs within the community.

Responsibilities

• Assess and interview clients to identify problems and goals.
• Coordinate with local authorities and organizations for resource allocation.
• Provide ongoing support and advocacy for clients in need.
• Organize and lead community outreach activities.
• Participate in interdisciplinary team meetings to discuss client cases.
• Document service provision and report on client outcomes.
• Stay informed about relevant legislation and policy changes, ensuring compliance.

Requirements

Education

• Degree in Social Work or a related discipline
• Master’s degree preferred

Experience

• Minimum 2 years in a social work setting
