Job Description

We are seeking compassionate and dedicated Social and Human Service Assistants to join our team in Manchester. You will be responsible for assessing the needs of vulnerable individuals, developing care plans, and providing emotional and practical support. This role is vital in helping our clients regain independence and enhance their wellbeing.

Responsibilities

  • Conduct regular assessments to identify client needs.
  • Develop clear care strategies with measurable goals.
  • Support clients with daily activities and personal care.
  • Document and report any changes in clients' situations.
  • Collaborate with multidisciplinary teams to enhance service delivery.
  • Evaluate the effectiveness of care plans and make necessary adjustments.
  • Facilitate access to additional resources for clients.
  • Educate clients on health and wellbeing practices.
  • Promote independence through skills training and support.
  • Represent the organization at community outreach events.

Requirements

Education
  • Level 3 Diploma in Health and Social Care or equivalent
  • First Aid certificate (desirable)
Experience
  • No formal experience required, but experience in a caring role would be beneficial.
