Responsibilities

• Lead patient education efforts on health management and prevention.
• Support multidisciplinary healthcare teams to enhance treatment outcomes.
• Document assessments, plan interventions, and follow ups in patient records.
• Facilitate referrals to specialists and coordinate patient care.
• Stay informed on current healthcare trends and best practices.
• Participate in training and mentorship of junior healthcare staff.

Requirements

Education

• Master's degree in Nursing
• Additional certification in a specialty area is preferred

Experience

• 3+ years of experience in a healthcare setting

Certifications
