Job Description

We are currently searching for a dedicated Social Work Associate to support our social workers in enhancing the welfare of service users. This role involves assisting in the implementation of care plans and providing vital updates on the service users' conditions, ensuring they receive the best care possible.

Responsibilities

• Provide assistance with the execution of care plans designed for clients
• Offer consistent updates regarding service users' conditions and needs
• Be a part of case conferences and actively engage in multidisciplinary meetings
• Conduct thorough assessments and regular reviews of clients' progress
• Ensure thorough documentation and record-keeping of client information
• Supply practical support tailored to service users' individual needs
• Work collaboratively with external agencies to enhance care provisions
• Contribute ideas during the planning and updating of service user care plans
• Support social work professionals in their regular operational tasks
• Engage in ongoing learning and training programs for skill enhancement

Requirements

Education

• Relevant qualification in social work or a related field

Experience

• At least 2 years of experience in supporting vulnerable adults or children

Languages

• English: Fluent

Advantageous
