Job Description

The Public Health Social Worker will collaborate with health professionals and stakeholders to enhance the wellbeing of individuals, families, and communities by addressing the social determinants of health. The role encompasses assessments, counseling, health promotion, and advocating for those in need.

Responsibilities

  • Conduct thorough assessments to identify the social factors affecting health.
  • Create and implement tailored care plans based on assessments.
  • Provide education and resources to families regarding health issues such as mental health.
  • Network with local organisations to foster collaborations that enhance community health.
  • Monitor and record each client's progress and community health initiatives.
  • Facilitate workshops and awareness programmes focusing on public health.
  • Support clients in accessing additional health and social services.

Requirements

Education
  • BA/BS in Social Work, Public Health, or a related field
  • Registered with the Health and Care Professions Council (HCPC)
Experience
  • Minimum of 2 years of experience in a social work or public health role.
