Job Description

As a Hospice Admitting Clerk, you will be the first point of contact for patients and families seeking admission to our hospice. Your role is vital in guiding individuals through the admission process while ensuring they feel supported and respected. The ideal candidate will demonstrate exceptional communication skills and a deep understanding of the emotional complexities faced by families during this time.

Responsibilities

  • Oversee the admissions process, ensuring that all patient information is collected accurately.
  • Liaise with care teams to coordinate admissions timely.
  • Offer support to families during the admission process, addressing concerns and queries.
  • Create and manage electronic patient records and files.
  • Review and adhere to hospice policies and ensure quality care is provided.
  • Conduct follow-ups on pending admission paperwork.

Requirements

Education
  • Relevant healthcare qualification
Experience
  • Experience in hospice or palliative care settings is advantageous.
Technical Skills
  • Customer Relationship Management (CRM) software
Soft Skills
  • Attention to detail
  • Ability to work under pressure
