Job Description

As a Nurse Practitioner in Swansea, you will be responsible for assessing, diagnosing, and managing health conditions of patients within your care. Your expertise will ensure that we continue to provide outstanding healthcare services in our community.

Responsibilities

  • Evaluate patients' medical histories to identify health concerns.
  • Manage patient care in a variety of settings, including clinics and home visits.
  • Educate patients on preventive healthcare and lifestyle changes.
  • Collaborate with physicians to plan and implement patient care.
  • Conduct follow-up appointments to monitor patient progress.
  • Assist in developing healthcare policies and protocols.
  • Engage in community outreach programs to promote healthcare services.
  • Document patient interactions in accordance with legal and ethical guidelines.
  • Provide mentorship to new staff and interns.
  • Lead patient care initiatives and assess their effectiveness.

Advantageous

  • Experience in geriatric care: Knowledgeable in treating elderly patients with complex health needs.
  • Additional certifications such as ACLS or PALS: Certification in Advanced Cardiovascular Life Support or Pediatric Advanced Life Support.
