Job Description

Responsibilities

  • Guide individuals in understanding their health rights and accessing appropriate services.
  • Perform detailed assessments to tailor care plans to individual needs.
  • Work alongside healthcare professionals to ensure integrated patient care.
  • Conduct workshops to empower community members with health knowledge.
  • Track and maintain detailed documentation of health interventions and outcomes.
  • Organise and lead health promotions in collaboration with local organisations.
  • Educate clients about available health resources and services in the community.
  • Support management in evaluating the effectiveness of health programs.
  • Provide crisis intervention support when necessary.
  • Foster relationships with community stakeholders to improve service delivery.

Requirements

Education
  • Bachelor's degree in Healthcare, Social Sciences, or related field
  • Additional certifications in community health or case management are advantageous
Experience
  • At least 2 years of experience in a similar role within a community health setting
