Job Description

We are seeking a highly organized and compassionate Discharge Planner to join our healthcare team. The Discharge Planner will be responsible for coordinating and facilitating smooth patient transitions from hospital to home or other healthcare facilities. The ideal candidate will have excellent communication skills, a strong understanding of medical terminology, and the ability to collaborate effectively with interdisciplinary teams.

Responsibilities

  • Assess patient needs and develop personalized discharge strategies.
  • Coordinate care with multidisciplinary teams to ensure smooth transitions.
  • Educate families on post-discharge care and community resources.
  • Document discharge plans and maintain patient records diligently.
  • Collaborate with insurance and case managers for resource availability.

Requirements

Education
  • Bachelor's degree in Healthcare Administration or related field
  • Registered Nurse (RN) or Social Worker (SW) license preferred
  • Certification in Case Management (CCM) is a plus
Experience
  • Minimum of 2 years of experience in discharge planning or a related healthcare role.
Technical Skills
  • Excellent communication and interpersonal skills
