Job Description

We are excited to announce an opening for a Nurse Practitioner in Cardiff. In this role, you will provide comprehensive care to patients, working closely with a multidisciplinary team. Your expertise will be pivotal in enhancing patient outcomes and promoting health education.

Responsibilities

  • Perform comprehensive initial and ongoing assessments.
  • Manage healthcare needs of a diverse patient population.
  • Provide counselling and support for mental health issues.
  • Coordinate care with specialists and allied health professionals.
  • Conduct health promotion and disease prevention activities.
  • Facilitate patient access to necessary healthcare resources.
  • Document and track patient progress effectively.
  • Educate staff and community on health issues.
  • Utilize technology for patient management and communication.
  • Participate in continuing education and professional development.

Requirements

Education
  • Master's degree in Nursing or equivalent
  • Doctor of Nursing Practice (DNP) preferred
Experience
  • 5+ years of advanced practice nursing experience preferred.
